For Private Sector Employees

For Public Personnel

A duty letter from the institution he/she is affiliated with (stamped and signed) and the dates between which he/she started working will be stated.
Permission letter from the institution he/she was affiliated with (stamped and signed)
Last 4 months salary slip
Bank book processed and with balance for the last 3 months
Credit card statements
Travel health insurance
Petition explaining the purpose of travel

Retirement book
Document showing the institution where he/she received his/her last 4 months salary or bank account book.
