The following requests concern visas to be applied by the Consulate General of the Czech Republic in Istanbul. A valid passport must be valid for at least 3 months after the expiration date of the passport and must have a blank page for the visa; the photograph in the passport must be the same as the person's appearance on the application date. *Passport Photocopy - photocopy of the passport's picture page, validity period page, identity information page, back page and Schengen visas obtained within the last six months. *Original application form for each person traveling (including minor children traveling together), legibly and completely filled in and signed by the passport holder. The form must be filled in capital letters, one letter in each box, and nothing must be written outside the boxes. *1 photo - the size of the photo (3.5 x 4.5 cm) must be on a light background, the face and upper shoulders must be visible, the full image must be 70% of the photo, looking straight ahead. The appearance in the photograph must match the applicant's appearance. *Travel insurance covering the entire duration of the trip (minimum 30,000 euros, covering sudden illnesses, injuries and return expenses to the country). In case of a short-term multiple entry visa, a declaration stating that insurance will be provided for subsequent trips after the first trip must be signed. Travel insurance must be arranged to start at least one day before the start of the trip and end one day after the end of the trip.
